Private/Group Health Insurance
Performance Rehabilitation accepts all major insurance plans including Medicare. We are preferred providers for most group health insurance plans. It is our policy to confirm the eligibility and benefits of all of our patients and inform them of their coverage. We also contact the insurance company and obtain authorization for therapy when required.

Please bring your insurance card with you during your first visit so we can properly verify your benefits and file with your insurance company.

Worker's Compensation
Performance Rehabilitation has a strong background in working with workers injured on the job. Injured workers are covered through their employer's worker's compensation insurance plans. Performance Rehabilitation will provide objective functional data to the physician, employer and insurance company to aid in an efficient and safe return to work.

Personal Injury/Liability and Auto Accidents
Performance Rehabilitation gladly accepts patients involved in personal injury or automobile accidents. A financial agreement will have to be signed during the patients first visit.
